For this new service, the post holder will provide support to families of young people aged 14+ with additional support needs as they navigate the transition to adulthood. The primary focus is on prevention, capacity building, and enhancing community and social connectedness. By intervening early and strengthening family capability, the service aims to reduce the likelihood of crisis escalation and promote sustainable independence outcomes for the young person. Engagement with the Parent/Carer Transitions Worker will help families develop greater confidence in supporting their young person’s journey toward independence, alongside an improved understanding of adult services, community resources, and available pathways. They will experience reduced anxiety and isolation, and increased resilience and day‑to‑day capacity, enabling them to provide more effective and sustained support.
