Background
Neighbourhood Networks is a registered Scottish Charity with 20 years' experience in facilitating peer support networks with vulnerable adults across the Central Scotland.We work with people who may be isolated or at risk of isolation within their communities. This may be because of a learning disability, poor mental health, a physical disability, sensory impairment or because of a variety of other circumstances.
The Role
For this new service, the post holder will provide support to families of young people aged 14+ with additional support needs as they navigate the transition to adulthood. The primary focus is on prevention, capacity building, and enhancing community and social connectedness. By intervening early and strengthening family capability, the service aims to reduce the likelihood of crisis escalation and promote sustainable independence outcomes for the young person. Engagement with the Parent/Carer Transitions Worker will help families develop greater confidence in supporting their young person’s journey toward independence, alongside an improved understanding of adult services, community resources, and available pathways. They will experience reduced anxiety and isolation, and increased resilience and day‑to‑day capacity, enabling them to provide more effective and sustained support.‘Together anything is possible’

Supporting families of young people aged 14 and over as they navigate the transition to adulthood, this role focuses on strengthening family capability through early intervention and preventative support, building confidence, resilience and everyday capacity within parent and carer networks, and promoting independence outcomes for young people by enhancing their community and social connectedness.
Values, Approach and Workload
Key Accountabilities and Tasks
• Provide one‑to‑one support to parents and carers, offering guidance, emotional support, and practical strategies
• Facilitate parent/carer peer support groups, both in‑person and online
• Deliver workshops on topics such as independent travel, money management, digital safety, adult services, and community pathways
• Support families to understand adult services, transitions processes, and realistic expectations of statutory support
• Strengthen community connections by introducing families to local organisations, resources, and informal networks
• Reduce isolation by encouraging participation in community activities and peer networks
• Collaborate with partner agencies including HSCP, education, social work, and third‑sector organisations
• Contribute to referral pathways, triage processes, and early‑intervention planning
• Maintain accurate records, safeguarding responsibilities, and monitoring information
• Gather evaluation data, including case studies, feedback, and outcome measures
• Promote preventative approaches that reduce crisis escalation and inappropriate referrals to social work
